Every five years Anoka County Public Health and Environmental Services (PHES), updates its statutorily required community health assessment and plans. The plans address how PHES works with community partners to promote and protect the health of Anoka County residents.
2018-2019 Community Health Assessment
Anoka County Public Health and Environmental Services is partnering with healthcare providers, non-profit organizations, and others to identify the most important health needs in our community. This information will help the community develop strategies to address these health issues and make Anoka County a place where all can be healthy. A recent survey of adult residents of Anoka County provides valuable information about health challenges in our community. The following summary reports describe results from the survey.
