If you are from a law firm, insurance company, or other private entity please send a written request on your letterhead to our office. The request may be sent via U.S. Mail, fax, or email.
Include in Request:
Name of the decedent
Date and county of death
What you are requesting
Where the records should be sent
We also require an authorization form signed by the legal next of kin or a court order to release the records. Once the request is received, our office will correspond regarding any fees that apply. If the case is not complete we will keep your request on file until the records are ready to be released.
