Community First Services and Supports (CFSS)

Community First Services and Supports Brochure (PDF) - CFSS is a Minnesota healthcare program designed to provide flexible options tailored to the unique needs of individuals. It promotes greater independence for people within their homes and communities. CFSS will replace the Personal Care Assistance (PCA) program and the Consumer Support Grant (CSG).

CFSS Rights and Responsibilities (PDF) - Individuals using CFSS have rights that must be respected and protected by their support workers, providers, lead agency staff, and the Department of Human Services (DHS). This document outlines the rights and responsibilities of the individual, lead agency, and provider agency.

Anoka County Senior Housing Guide (PDF) - Customized living can be an apartment, room, or companion suite with available services such as housekeeping, laundry, personal cares, activities, and meals. It is designed for those who need extra assistance in their day-to-day lives but who do not require nursing home care. Some facilities offer memory care, which provides specialized staff and programming for those living with dementia or Alzheimer’s disease.
